Please click the file below to download the OIWC Luncheon Reservation Form. Submit a completed copy along with your payment to one of our drop box locations.

How to use a Drop Box

Put cash inside a sealed envelope with either a membership form or reservation form. If you completed the form using a QR code, ensure the envelope is complete with all information.
Label the outside of the envelope with your name, contact number, what and who you are paying for as well as culture group name.
Include exact amount in US Dollars for on-base luncheons or Japanese Yen for off-base luncheons; all excess will be donated. Please note, each event changes between USD and Yen and required payment type is notated on all advertisements.
Place envelope into red lock box labeled OIWC at one of the below locations. DO NOT leave an envelope if the box is not present.
Cancellations for luncheons made after the deadline will result in loss of money (see Cancellation Policy below).
When using the QR code to complete luncheon reservations, monies must be received by the due date and time. Just simply filling out the QR code form does not guarantee a reservation.

Foster:
990 C
From Gate 7 (Hospital Gate: Enter the gate and take your first right. Take your first left into the neighborhood and then your first right. 990 C will be in the second row of townhouses on your right.

Kadena:
2078 D Walmsley Way
From Gate 5: Turn right at first stop light onto Vincent Ave, heading towards Kadena High School, Continue on Vincent Ave until you approach the Rocker Enlisted Club, turn right at Walmsley Way and follow road until last building, 2078 on the right side.

From Gate 3: Go straight through gate onto Douglas Blvd, turn left at the Fairchild intersection (near the Express). Follow road until stop sign at Vincent Ave (corner of Ryukyu Middle School/Bechtel Elementary). Turn right and follow road past Rocker Enlisted Club. Turn left onto Walmsley Way and follow road until last building, 2078 on the right side.

Courtney:
4459
From Gate 1: Go straight to the stop sign and turn right onto Jackson Street. Take the first left and Building 4459 will be the house at the end of the street on the right.

Plaza:
4200
From Plaza Housing Gate: Continue on the road and follow the right hand curve. Take the first left to the dead end and then turn left up the hill. End at building 4200 (there is a flag pole that can be seen from the road).
